ABSTRACT

BACKGROUND: Helicobacter pylori colonizes the gastric surface epithelium and the mucus gel layer. It has been known that H. pylori infection decreased the gastric mucin expression. The aim of this study was to determine the effect of H. pylori eradication on mucin expression (MUC5AC, MUC6 and MUC1) in the gastric epithelium. METHODS: This study included 20 patients positive for H. pylori whom successful eradication was performed between March 1998 and December 1999. H. pylori status was determined by histology, rapid urase test and urea breath test. Gastric antral biopsy specimens were examined by immunohistochemistry for mucin (MUC5AC, MUC6 and MUC1) expression. The distribution of epithelial cells expressing MUC5AC was calculated at two sites (surface mucous cells, pyloric glands). Two scores system (weak-strong) was used to assess staining intensity. RESULTS: There was a gradient of MUC5AC expression, higher to lower from the surface to the glands. Increased MUC5AC expression in the surface mucous cell (p=0.013) and in the glands (p=0.008) was found after H. pylori eradication. MUC6 and MUC1 distribution was not changed after H. pylori eradication. CONCLUSION: MUC5AC expression was increased after H. pylori eradication. These results suggest that MUC5AC may relate in the pathogenesis of H. pylori.

ABSTRACT

PURPOSE: We intended to evaluate the results of arthroscopic anterior cruciate ligament(ACL) reconstruction using Bone-achilles tendon-bone composite allograft by analyzing the results clinically and radiologically. MATERIALS AND METHODS: From May 1996 to June 1999, we followed 23 patients who had arthroscopic ACL reconstruction using bone-achilles tendon-bone composite allograft for minimum 24 months, and measured laxity using KT-2000 arthrometer, Lysholm score and enlargement of tibial tunnel width on plain radiograph. RESULTS: In KT-2000 arthrometer test, there was only side-to-side differences of 1.67 +/-0.7 mm between the involved and contralateral limb. Lysholm score was 61.7 +/-10.2 preoperatively and 89.3 +/-5.1 at the last follow up. And there was no tibial tunnel enlargement more than 5 mm at the last follow up. CONCLUSION: Arthroscopic ACL reconstruction using bone-achilles tendon-bone composite allograft has advantage of preservation of anatomic structure and secure bone-to-bone fixation like the bone-patel-lar tendon-bone graft that bone-achilles tendon-bone composite allograft is a reasonable choice in ACL reconstruction.

ABSTRACT

BACKGROUND: Typhoid can be diagnosed with certainly only by the isolation of Salmonella Typhi from the blood, urine, feces, or other body fluids. The Widal reaction has been one of the most important diagnostic tests for typhoid and is still widely used in many developing countries. METHODS: The Widal test was done in 50 nontyphoidal febrile patients, and 50 patients with proven typhoid in Chonbuk area. we compare this result with that of last 10 years. RESULTS: By rapid slide method S. typhi O titer of 1:160 or more was seen in 58% and S. typhi H titer of 1:320 or more was seen in 12% of patients with bacteriologically proven typhoid fever. CONCLUSION: O titer was similar to the result of past 10 years. A false positive test was found in 40% of patients with nontyphoidal febrile patients in whom Widal test must therefore be interpreted with caution.

ABSTRACT

Autogenous bone-patellar tendon-bone is popular ligament substitute used in anterior cruciate liga- ment(ACL) reconstructions. Fracture of the patella is a rare but serious complication unique to this type of reconstruction. We report 4 cases of patellar fracture after autogenous bone-patellar tendon-bone ACL reconstruction.

ABSTRACT

A 46-year-old male patient developed jaundice after 23 days' treatment of propylthiouracil (PTU) for Graves' disease. Serum alkaline phosphatase was elevated markedly with moderate increase in serum aminotransferase, and the peak level of total bilirubin was 7.3 mg/dL. After withdrawal of PTU, serum aminotransferase and bilirubin began to decrease simultaneously, and completely normalized. One week after the discharge from hospital, he received radioiodine treatment for Graves' disease, but he showed aggravation of hyperthyroidism and ophthalmopathy. So we prescribed methimazole inevitably 16 days' after the radiodiodine treatment. He developed jaundice again after 69 days' treatment of methimazole, but the pattern of hepatotoxicity was slightly different from that of the previous PTU-nduced hepatotoxicity. Serum aminotransferase increased slightly and quickly normalized after discontinuance of methimazole, but serum total bilirubin increased continuously and reached to 24.6 mg/dL. Eosinophilia was prominent and the result of liver biopsy was compatible with cholestatic jaundice. The patient was treated with prednisolone and recovered from jaundice rapidly.

ABSTRACT

Ischemic colitis represents most common form of gastrointestinal ischemia, usually developed in elderly or debilitated patient with a variety of underlying medical problems. The presumed etiologies are numerous. It usually presents as an acute abdominal illness with bloody diarrhea. Subacute ischemic colitis in healthy adults develops spontaneously in the absence of major vasculature occlusion, and the original insult precipitating the ischemic event usually cannot be established. Diagnosis is based on the clinical symptom, barium study, colonoscopic and histopathologic findings. Therapy and outcome are depen-dent on the severity of disease. It usually requires only medical management and is asso-ciated a good prognosis. We report a case of subacute ischemic colitis in healthy adults developed after severe watery diarrhea with literatures.
